Parents / Child Questionnaire

Optimum school performance requires optimum visual function. Our examination goes beyond the traditional sight examination, to investigate how comfortably and efficiently your child’s focussing and binocular vision works. We assess vision information processing, to gain an insight as to whether your child understands what he/she sees. Stress on any aspect of vision can lead to development of adaptations, both functional (myopia development, turned/lazy eye, postural anomalies) and behavioural (avoidance, poor concentration). We evaluate for these adaptations and plan a course of action to return visual development to normal pathways.

Knowing your child’s history, and gaining an insight through observations made by yourself and others, allows us to understand where your child is situated, in terms of their development. We aim to provide your child the best chance to develop an efficient, comfortable vision system that will allow your child to learn to their full potential.

Out testing for Visual Information Processing is inclusive of testing components of Dyslexia which includes: Dyspraxia, Laterality, Dysnemkinesia, Dyseidesia, Dysphonesia and Auditory Processing. We also assess for Irlen-Meares Syndrome, however precise determination of any spectral-coloured lens filter will require a subsequent consultation.

Subsequent to the testing a written report (up to 3 copies) will be prepared including age related performance comparisons. We request 30 minutes of parental time to fully discuss the findings in the report 1 week after the initial assessment. (Report discussions do not incur further fees) Reports may be given to teachers, speech therapists, chiropractors, occupational therapists, and general practitioners as required.

Full comprehensive assessment takes up to 2+ hours to complete. Cost is $325.00. If a full Medicare rebate applies, $86.60* will be refunded by Medicare. Further testing at any subsequent visits may attract other charges: (e.g. colourimetry, visual field assessment, ocular coherence tomography).

Standard optometric evaluation with binocular vision work-up but without full assessment of vision processing (does not include a report), takes 30 to 40 minutes and costs $195.00. If a full Medicare rebate applies, $86.60* will be refunded by Medicare
